***Certification AND Documentation of Abatement Required***
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were causing or
likely to cause death or serious physical harm to employees in that
employees were exposed to:
the hazard of being struck by or crushed by the equipment or material
being worked due to the
unexpected energy released by the force of gravity.  On April 4, 2004 at
4800 Old Port Industrial Blvd.,
Galveston, Texas, employees structurally weakened the supporting structure
of the mobile conveyor boom
spout when removing two "X" braces and the top two, ten foot sections of
3"X3"X1/2" structural
supports.  The piece of equipment was not properly supported
causing structural failure/collapse.
Among other methods,one feasible and acceptable abatement method is to:
(a) to implement a
lock out/tag out procedure as prescribed in Section 3.1.2 of ANSI
Z244.1-1982,
American
National Standard for Personnel Protection - Lockout/Tagout of Energy
Sources - Minimum
Safety Requirements.  (b) lowering the conveyor boom to a horizontal
position and adding
adequate support under the boom.
