Title 40.1-51.1.(a), Code of Virginia:  The employer did not furnish
employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in
that:
(a) Employer exposed employees to fall hazards of 13-40 feet,  by allowing
mobile
scaffold height to
exceed 4 times maximum height to base dimension restrictions as required.
Employees were permitted to use Mobile
scaffold with 2 foot frames, 13 feet high and greater. Outriggers were not
utilized. This occurred while employees
were conducting overhead cleaning operations located in building L2 of the
Upper Occoquan Sewerage Authority plant
located at 14631 Compton Rd. Centreville. " One feasible and acceptable
abatement method, among others, to correct
this hazard is to install outriggers to provide additional stability".
