Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by an unsecured compressed gas carbon dioxide cylinder: a)Computer room: The carbon dioxide cylinder used with the Gardner Bender Jetline conduit fishing system was not secured to prevent it from being knocked over, on or about 8/5/2003. Among other methods, one feasible and acceptable abatement method to correct this hazard is to; 1. Follow the Compressed Gas Association Pamphlet P-1-1965, in that section 3.4.4 states Before using cylinder, be sure it is properly supported to prevent it from being knocked over. 2. Use a suitable cylinder truck, chain, or other steadying device to keep cylinders from being knocked over while in use.e
