Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by an air hose and coupling: a)Stairway #1 and Stairway #2: The pneumatic hammer air hoses fittings did not have a coupling hold tight wire or pin in place, on or about 8/26/2003. Among other methods, one feasible and acceptable abatement method to correct this hazard is to : 1: Install and ensure that lock pins or hold tight wires are placed on the hose coupling.
