Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the following conditions:
City of Houston 48-inch water main construction site at Westview and
Beltway
8, Houston, TX:
On or about September 9, 2003, wood blocks used to support outriggers for a
Grove RT 58D, Serial #73557, were inadequate.  The wood block was crushed
and split, exposing employees to the hazard of struck by from a section of
pipe.
Among other methods, recognized and feasible means of abatement to correct
this
hazard include, but are not limited to:
The use of wood blocks strong enough to prevent crushing as required by the
American National Standards Institute (ANSI) standard B30.5-1968 (Crawler,
Locomotive and Truck Cranes), section
