Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer does not furnish employment free from the recognized hazard of employees placing their hands, arms and upper body parts into the operating work envelope of Articulated and Gantry Robots and colliding and becoming entangled with the moving robot parts, which is likely to result in serious amputation, puncturing, crushing and/or laceration injuries: a) Manufacturing Department, ABB Articulated Robot Machine Nos. 4591, 4675 and 4701 ! Employees operating ABB Articulated Robot Machine Nos. 4591, 4675 and 4701 were exposed to the recognized hazard of placing their hands, arms and upper body parts into the operating work envelope of Articulated Robots to load cutting stations with fiberglass mats that are to be cut and trimmed by the water jet tool affixed to the robot, to remove finished pieces, and to remove scrap fiberglass mat material left over after cutting, permitting the employees to become entangled with the moving robot parts, which is likely to result in serious amputation, puncturing, crushing and/or laceration injuries. This hazard was present on or about May 12, 2004 when the presence sensing mats which were designed to stop the robot if an employee stepped into the work envelopes of these robots had been disconnected. b) Manufacturing Department, ABB Articulated Robot Machine Nos. 4686, 4700, 4703 and 4704 ! Employees operating ABB Articulated Robot Machine Nos. 4686 4700, 4703 and 4704 were exposed to the recognized hazard of placing their hands, arms and upper body parts into the operating work envelope of Articulated Robots to load cutting stations with fiberglass mats that are to be cut and trimmed by the water jet tool affixed to the robot, to remove finished pieces, and to remove scrap fiberglass mat material left over after cutting, permitting the employees to become entangled with the moving robot parts, which is likely to result in serious amputation, puncturing, crushing and/or laceration injuries. This hazard was present on or about May 12, 2004 when the work envelopes for these robots were not protected by any presence sensing mats, barriers guards, light curtains or any other equally effective means. c) Manufacturing Department, Robotics, Inc. Gantry Robot Machine Nos. 4501, 4590 and 4601 ! Employees operating Robotics, Inc. Gantry Robot Machine Nos. 4501, 4590 and 4601 were exposed to the recognized hazard of placing their hands, arms and upper body parts into the operating work envelope of Gantry Robots to load cutting stations with fiberglass mats that are to be cut and trimmed by the water jet tool affixed to the robot, to remove finished pieces, and to remove scrap fiberglass mat material left over after cutting, permitting the employees to become entangled with the moving robot parts, which is likely to result in serious amputation, puncturing, crushing and/or laceration injuries. This hazard existed on or about May 12, 2004 when the light curtains which were designed to stop these robots if an employee interrupted the light beam had been disconnected. A feasible means to abate the recognized hazard of employees placing their hands, arms and upper body parts into the operating work envelopes of each of the Articulated and Gantry robots in the Manufacturing Department is to install fixed barrier guards or other safeguarding devices such as light curtains or presence sensing mats that either prevent employees from accessing the operating work envelope or automatically shut down the robot in the event that the operating work envelope is penetrated. Safeguarding devices shall be selected and installed in accordance with ANSI R15.06-1999 ! Safety Requirements for Robots and Robot Systems.ents

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer does not furnish employment free from the recognized hazard of employees entering into the restricted work envelope of Articulated robots and colliding and/or becoming entangled with the moving robot parts, which is likely to result in serious amputation, puncturing, crushing and/or laceration injuries: a) Manufacturing Department, ABB Articulated Robot Machine Nos. 4700, 4701, 4703 and 4704 ! Employees operating ABB Articulated Robot Machine Nos. 4700, 4701, 4703 and 4704 were exposed to the recognized hazard of entering into the restricted work envelope of Articulated robots and colliding with the moving robot, becoming entangled with the moving robot and/or becoming entrapped between the moving robot and fixed articles such as the cutting basin, inspection tables and the robot control panel , which is likely to result in serious amputation, puncturing, crushing and/or laceration injuries. This hazard existed on or about May 12, 2004 when portions of the restricted work envelopes of these robots were not protected by any barriers, mechanical stopping devices, or electronic limit switches. A feasible means to abate the recognized hazard of employees entering into the restricted work envelope of each of the Articulated robots in the Manufacturing Department is to either permanently barricade any portion(s) of the work envelope that extends beyond the footprint of the cutting basins so that employees cannot enter this area or install limit switches or mechanical stops which will prevent the robots from traveling past the footprint of the cutting basin in the event of a malfunction. Establishment of the Restricted Work Envelope shall be performed in accordance with ANSI R15.06 ! Safety Requirements for Robots and Robot Systems.ents

Gantry Robot Machine Nos. 4472 and 4480 -- Employees operating the Robotics, Inc. Gantry Robot Machine Nos. 4472 and 4480 were exposed to recognized hazard entering the path of the automatic indexing tables of Gantry Robots and become crushed between the automatically traveling indexing tables and the table stops. This hazard existed on or about May 7, 2004 when the work envelopes of these robots were not protected by any presence sensing mats, barriers guards or light curtains designed to stop the robot if an employee entered the work envelope. A feasible means of abating the recognized hazard of employees entering the path of the automatic indexing tables on the Gantry robots in the Manufacturing Department is to install fixed barrier guards or other safeguarding devices such as light curtains or presence sensing mats that either prevent employees from accessing the area of the tables or automatically shut down the tables in the event that the area surrounding a table is penetrated. Safeguarding devices shall be selected and installed in accordance with ANSI R15.06- 1999 ! Safety Requirements for Robots and Robot Systems.ents

Gantry Robots, including but not limited to Machine Nos. 4701, 4591, 4686, 4704, 4703, 4675, 4700, 4590, 4601 and 4501 ! Employees operating the ABB Articulated and Robotics, Inc. Gantry Robots, including but not limited to Machine Nos. 4701, 4591, 4686, 4704, 4703, 4675, 4700, 4590, 4601 and 4501 were exposed to the recognized hazard of placing their hands, arms and upper body parts into the path of the 40,000 pounds per square inch cutting water jets of Articulated and Gantry Robots. This hazard existed on or about May 12, 2004 when the points of operation of the jets on these robots were unguarded. A feasible means to abate the recognized hazard of employees placing their hands, arms and upper body parts into the path of the water jets affixed to each of the Articulated and Gantry robots in the Manufacturing Department is to install fixed barrier guards or other safeguarding devices such as light curtains or presence sensing mats that either prevent employees from accessing the operating work envelope or automatically shut down the robot and water jet in the event that the operating work envelope is penetrated. Safeguarding devices shall be selected and installed in accordance with ANSI R15.06-1999 ! Safety Requirements for Robots and Robot Systems.
