North Carolina General Statute 95-129(1) of the Occupational
Safety and Health Act of North Carolina:  The employer did not
furnish each of his employees conditions of employment and a
place of employment free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees in that employees were exposed to:
a)facility -  chain hoist attachment, and building
supports for lifting parts were not designed and rated
to withstand the loads and forces imposed by the parts
being lifted. (Reference ANSI 30.16 -paragraph
1.2.1.,design, paragraph 16.1.3, installation,
paragraph 16-2.1.1, inspection., also ANSI B30.20-1999
sections 20-1.2.1, markings, section 20-1.2.2

Among other methods, one feasible and acceptable
abatement method to correct this hazard is to procure a
structural engineering certification for the load
rating of the structure support beam, and chain
attachment.
