Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Punch area - The Parker DD2HV hydraulic punch press, serial number EE058956A, the two- hand controls were not protected against unintentional operation with ring or shroud guards. b) Punch area - The Parker Hannifin hydraulic punch press, Model A-3L 14, serial number AA62091A, two-hand controls were not protected against unintentional operation with ring or shroud guards. Among other methods, one feasible and useful method to correct this hazard is to follow ANSI B11.2 - 1995 - Section 6.3.9(1), which states When two-hand control is provided, each hand control shall be protected against unintended actuation. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).s
