Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of amputation and fracture: (a) Press Brake Area- On the Amada RG-50 hydraulic power press brake, Serial Number 50- 00112, the means of fixing the selection of modes between "off", "inch", and "single stroke" were not being supervised by the employer. (b) Press Brake Area- On the Cincinnati press brake, Serial Number 46273, the means of fixing the selection of modes between "off", "inch", and "single stroke" were not being supervised by the employer. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)
