North Carolina General Statute 95-129(1) of the Occupational Safety
and Health Act of North Carolina:  The employer did not furnish
employment and a place of employment which were free from
recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that employees were exposed
to a struck-by hazard from projectile fragments of a disintegrating
coupling sleeve:
a)mill #2, Tierney Bottom Arbor Gang saw (Bull edger), on
4/28/2003 the Bull Edger was stalled while running a
hickory cant, causing the coupling sleeves to come apart
and be thrown from the coupling assembly, striking an
employee. A caution was included in the installation
instructions for the TB woods coupling stating "coupling
sleeves may be thrown from the coupling assembly with
substantial force when the coupling is subjected to a
severe shock load."
Among other methods, feasible and acceptable abate
methods to correct this hazard are:
1)Train the operator that shock loading the
"Bull Edger" can cause the coupling sleeves to
be thrown from the coupling assembly, the
signs of an impending stall, and the proper
methods to prevent or control a stall.
2)Reduce the feed rate on the intake side of the
Bull Edger.
3)Construct a guard for the coupling that will
completely enclose the coupling assembly.
