Section 88.4 - Code of Iowa (2003) 88.4: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards created from the use of unapproved modifications to the hydraulic lift: (a)In the Grease Room - Employees are using a hydraulic lift with extensions that were not made by the manufacturer of the hoist. The employer did not have documentation to show the manufacturer of the hoist had given permission to modify the use of the hoist. Among other methods, one feasible and acceptable method to correct this hazard is to discard the unauthorized extensions, or obtain extensions that are made by the manufacturer of the hoist, if any exist. ANSI B153.1 - 1990, Section 7.1.3 states: Modifications to any lift shall not be made without the prior written consent of the lift manufacturer. Automotive Lift Institute Standard for Automotive Lifts - Safety Requirements for Operation, Inspection and Maintenance, Section 7, also requires any modifications to the equipment be approved in writing by the manufacturer.

Section 88.4 - Code of Iowa (2003) 88.4: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries and death from the potential failure of the two hydraulic automotive lifts due to the lack of an inspection program that would identify any deficiencies and other hazards: (a)At the Site - The employer had two hydraulic automotive lifts that were not included in a periodic inspection program. Employees use the lifts on a routine basis. Among other methods, one feasible and acceptable method to correct this hazard is to have a qualified person, knowledgeable of the automotive lift inspection requirements conduct the necessary inspections. The inspection requirements are detailed in the manufacturers manual or the Automotive Lift Institute Standard for Automotive Lifts -1994, Section 5 and Appendix - C, Periodic Inspection Checklist. ANSI B153.1 - 1990 also requires periodic inspections of the automotive hydraulic lifts.
