employees in that employees were exposed to hazards associated with
confined space entry:
(a)Storage Tank - There was no confined space survey, hazard
identification &
evaluation, written program, training or atmospheric testing to ensure
safe entry
into confined spaces. This resulted in a fatality on 7/18/03 where an
employee
suffocated due to the lack of oxygen. Among other methods, one feasible and
acceptable abatement method to correct this hazard is to implement a
confined
space program including survey of spaces, hazard identification and
evaluation,
a written program, training and atmospheric testing as described in ANSI
Z117.1-
1995 safety requirements for confined spaces.d
