Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to falling into the elevator shaft from a height of several
feet:
a)East Freight Elevator -  The door or gate was not equipped with a
car-door or
gate electric contact that locked the door in the closed position before
the elevator
could be operated and prevent the door from opening when the platform was
not
parked on a level. The freight elevator platform was 4 1/2 to 5 feet above
the 3rd
floor landing when a housekeeper bypassed the door safety device and fell
approximately 50 feet to the bottom of the elevator shaft, on or about
05/04/04.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard is
to:
1) Conduct inspections of the elevator to ensure that the doors and
interlocking devices are in
safe operating condition. 2) Follow the requirements of ANSI/ASME A17.1 -
Rule 204.4,
Passenger and Freight Car Doors and Gates, General Requirements, 204.4b
Car-Door and Gate
Electric Contacts.
