Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not provide employment and a place of employment which were free of recognized hazards in that employees were exposed to fall hazards during the construction of a television communication tower. Rockford, IL. Channel 23 Jobsite : A. Three employees' accessing a communication tower in excess of 300 feet were hoisted simultaneously and without the use of a personnel platform. B. Employees' riding and working from the load line were not protected at the rooster head with an anti-2 block device. C. Trial lifts, inspections, proof tests and prelift meetings were not documented. D. Wire ropes were not inspected on a daily basis. E. The hook attachment to the headache ball was not equipped with a locking latch. F. A registered professional engineer did not provide the lifting capacity calculations of the gin pole and were therefore not available at the site. Among other methods, a feasible and acceptable means of abatement to correct these hazards is to instruct and train workers on safe work procedures before initiating hoisting operations on communication towers by following guidelines established by OSHA Directive CPL 2-1.36.
