Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish a place of employment which was free from recognized hazards that
were likely to cause
death or serious physical harm to employees in that employees were exposed
to the potential of
contacting high speed cutting tools whose guards were not interlocked.
Prince Industries/ Fabricators work place.  On or about January 6, 2004,
employees were exposed to high speed cutting tools whose guards were not
interlocked in the following instances:
a.  HAAS BF4 did not have an interlock guard;
b.  Mazak Quick Turn 20 at location Fanuc 6 did not have an interlock;
c.  Fanuc Drill Mate T Model 10 at location Fanuc 3 did not have an
interlock;
d.  Fanuc Robodrill T10B at location Fanuc 4 did not have an interlock.
A feasible means of abatement for this hazard would be to install
interlocks on
the barrier guards of the machinery in accordance with ANSI B11.19-1990
CERTIFICATION REQUIRED FOR THIS ITEM.
