Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to injuries and/or death from falls: On or about March 29, 2004, an employee was standing on the first highest rung from the top and the top back rear rail straddling a 10 foot fiberglass "A" frame ladder while installing lighting for a security system. This did not provide a suitable and stable surface on which to stand and work. Among others, one feasible and acceptable method of abatement is to ensure that ladder usage meets the requirements of ANSI Standard A14.5 2000. NO DOCUMENTATION NOR CERTIFICATION REQUIRED FOR THIS ITEM
