Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which was free from recognized
hazards that were
causing or likely to cause death or serious physical harm in that
employees were exposed to fall
hazards and crushing hazards due to absence of hoistway gate interlocks or
locking devices,
unguarded or partially guarded hoistway-landing openings, and absence of
elevator car door:
Turk Furniture, LaSalle store - the Otis freight elevator was not
installed in accordance
with the American National Standard for Elevators and Escalators (ASME
A17.1-2000).
Deficiencies include, but may not be limited to, the following sections:
a. Hoistway gate(s) were not equipped with interlocks or mechanical
locking devices, as
required by Section 2.12, to prevent opening of the gate from the landing
side unless the
car is within the landing zone.
b. Hoistway vertical sliding gates on the first, second, and third floors
did not fully
enclose the hoistway-landing openings as required by Section 2.11. The
hoistway-landing
opening at the basement level was not provided with an entrance.
c. The elevator car was not equipped with a car door as required by
Section 2.14.
Feasible means of abatement include installation of gate interlocks, total
enclosures of
hoistway-landing openings, and installation of elevator car door per ASME
A17.1-2000
Code requirements.
DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR
THIS ITEM.
