Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were exposed to possible crush hazards from falling vehicles while working with defective automotive lifts that had not been properly inspected, repaired or taken out of service. Garage Area. a.Forward automotive lift - Employees used the lift with two broken swing arm safety locks. b.Rotary Swing-Arm automotive lift - Employees used the lift with one swing arm safety lock that did not properly engage. c.Drive-On automotive lift - Employees used the lift with a side safety lock that did not stay properly engaged. d.Periodic inspections - The employer did not ensure that a periodic qualified inspection of each automotive lift had been performed and documented at least annually, or more often if recommended by the manufacturer. (ANSI/ALI- ALOIM 2000, 5.4 - 5.5). e.Removal from service - Defective lifts were not removed from service by the employer (ANSI/ALI ALOIM 4.5). DOCUMENTATION AND CERTIFICATION REQUIRED FOR THIS ITEM Some feasible means of abatement would include: 1.Establish compliance with the American National Standards Institute/American Lift Institute Standard ANSI/ALI-ALOIM-2000 "Standard for Automotive Lifts - Safety Requirements for Operation, Inspection and Maintenance." 2.Have lift operators conduct daily or pre-shift inspections and develop mechanism for operators to report problems to management. 3.When problems with lifts are reported, ensure appropriate inspection, maintenance and repairs are implemented by qualified personnel. 4.Remove defective lifts from service until repaired and in safe working condition. 5.Develop method for conducting and documenting periodic inspections of all lifts by qualified personnel at least annually or more often if required by the manufacturer.
