Section 5(a) 1 of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: employees were exposed to struck-by/amputation hazards, whereas, a saw blade discharged from a Sanders Saw Model# 064574, Serial# CC0302769 a) I-78 between Exit(s) 6&7 Bloomsbury, New Jersey 08804. The employer failed to require that all employees that operate the Sanders Saw Model#064574, read and understand the, "Sanders Saws & Blades Q-6457 TD Concrete Saw Operations Manual" as indicated on page seven & eleven. One feasible and acceptable method among others to correct this hazard is to follow the manufacturers printed instructions as follows: Require that all employees that operate the Sanders Saw Model#064574, to torque the 5/8 mounting bolt to 125-175 foot pounds of torque" as required on page twenty-three of the "Sanders Saws & Blades Q-6457 TD Concrete Saw Operations Manual",
