failed to
require that all employees that operate the Sanders Saw Model#064574, read
and
understand the, "Sanders Saws & Blades Q-6457 TD Concrete Saw Operations
Manual" as indicated on page seven & eleven.
One feasible and acceptable method among others to correct this hazard is
to follow the
manufacturers printed instructions as follows:
Require that all employees that operate the Sanders Saw Model#064574, to
torque the
5/8 mounting bolt to 125-175 foot pounds of torque" as required on page
twenty-three
of the "Sanders Saws & Blades Q-6457 TD Concrete Saw Operations Manual",
