Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a being struck by a falling load:
a.On or about December 21, 2004, an employee directing the crane lift did
not
ensure that the load a lid was secured and properly balanced before it was
lifted
from the form to be placed on the ground, exposing employees to the hazard
of
being struck by the load.
Among other methods, a feasible means of abatement would be to follow the
American National Standards Institute B30.2.0-1967, Overhead and Gantry
Cranes, section 2-3.2.3a, which reads:  The appointed individual directing
the lift
shall see that the load is well secured and properly balanced in the sling
or lifting
device before it is lifted more than a few inches.
ABATEMENT CERTIFICATION IS REQUIRED.
