Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: At the paper mill #6 Extruder: The employer failed to require that employees ensure rolls of paper on the extruder deck were chocked, exposing employees to the hazard of being caught in between the extruder deck and the rolls of paper. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS: To ensure that employees comply with company safety procedures as outlined in BJA Ext 81 and BJA Ext 50, that require the extruder operator to chock rolls of paper on the deck, and the industrial truck operator to verify rolls of paper are chocked before approaching the deck and releasing rolls of paper from clamps.ng

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: At the paper mill, the #6 Extruder: The employer failed to require industrial truck operators to verify the rolls of paper were chocked on the deck of extruder prior to adding rolls of paper to the deck with a power industrial truck, exposing employees to a crushing hazard on or about July 5, 2003. AMONG OTHER METHODS ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS: To ensure the clamp truck operators comply with the company safety procedures as outline in BJA Ext 50 that requires the operator of the clamp truck to sound the horn before approaching deck and before releasing roll from clamps, verify deck is clear of personnel and verify chock is in place before approaching the deck.
