employment and a place of employment free from recognized hazards which
caused or were
likely to cause death or serious injury to employees:
An employee was observed working approximately 20 to 25 feet above grade
from an
aerial lift (Snorkel Lift), without the use of a personal fall arrest
system or fall restraint
system.
