Minn. Stat. 182.653 subd. 2:  The employer did not furnish to each
employee, conditions of
employment and a place of employment free from recognized hazards which
caused or were
likely to cause death or serious injury to employees:
Two individuals were observed working from surfaces that exposed them to
fall hazards
of approximately 20 feet, without the use of a personal fall arrest system
or another type
of approved fall protection.  One person was observed inside the basket of
an articulating
boom type lift without fall protection, and the other employee was
observed walking
down the extended boom of the articulating boom type lift without fall
protection.g
