In that three instances were observed of two employees operating forklifts
throughout the facility
without the use of seatbelts.
1. Toyota forklift #67002 had its seatbelt missing and was operated
without the seatbelt.
2. Toyota forklift #66986 had a malfunctioning seatbelt that could not be
used.
3. Clamp truck #37330 had its seatbelt missing.  Use of the clamp truck
was determined through
interviews.
*One possible method of abatement would be to replace the seatbelts that
were removed with
new seatbelts and to encourage employees to use the seatbelts.
