employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing and fall hazards while operating the vertical
reciprocating conveyor.
Outside loading/unloading area:
1.The vertical reciprocating conveyor at the ground and dock levels
exposed workers to crushing
and falling hazards.
2. The lift was not equipped with signage prohibiting riders.
3. The open end on the platform exposed workers to fall hazards.
A method among other methods to correct the conditions are as follows:
1. Install interlocked gates at the ground and dock levels.
2. Post a sign prohibiting riders on the reciprocating conveyor platform.
3. Install standard guardrails that are capable of swinging outward.
4. Ensure the conveyor meets applicable requirements for design,
construction,
inspection,
testing, maintenance, and operation as prescribed in ANSI B20.1 1999.
