Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing hazards associated with an inadequately secured
and braced bundle of
granite slabs in a container on a truck trailer:
300 Oak Street, Unit 920, Pembroke, Massachusetts:
On or about 10-24-03 a bundle of granite in a container on a trailer
tipped
over crushing and
seriously injuring a worker offloading the bundles.
Feasible and useful methods to correct this hazard, among other methods:
1) Inspect bundles and bracing prior to entering the trailer to ensure
that bundles are
straight and secure against tipping, sliding or falling.
2) As bracing is removed, ensure that the remaining bundles are secure
against tipping,
sliding or falling.
3) During the unloading process, ensure that the remaining slabs and
bundles are
adequately secured to prevent them from falling on the workers.
4) Request the bundles to be shipped on "A" frames for increased stability.
5) Develop written procedures to address safe offloading procedures and
methods to
secure loads against tipping, sliding or falling.

exposed to struck-by hazards associated with improper rigging and handling
of bundles of granite
slabs with untrained employees:
300 Oak Street Unit 920, Pembroke, Massachusetts:
On or about 10-24-03 workers were exposed to being struck by bundles of
granite during
offloading from a container on a truck trailer as a result of rigging
bundles without
proper training on safe handling, rigging equipment and proper use of
communication
and hand signals.
Feasible and useful method to correct this hazard, among other methods:
1) Provide training on safe handling, rigging equipment and proper use of
communication
and hand signals to all employees involved in the off-loading process.
2) Post illustrations of hand signals on the worksite and train employees
to
use them.
3) Ensure that employees have access to rigging manuals before the job
begins.
