lifting beam that was
approved by the engineer of record for the hoisting of 5 bar joist.  The
employees
were using
the lifting bar for hoisting 7 bar joists.
Location:  Job site.
1.  The lifting beam used to lift seven (7) bar joist at one time was
engineered to
lift five (5) bar joist at one time.
2.  The lifting beam being used was not built in accordance with the
specification
sheets in that ther were six (6) attachment points for connecting bar
joist to the
beam.  The specification sheet called for five (5) attachment points.
Among other methods one feasible and acceptable abatement method would be
to
Fabricate and use the lifting beam as approved by the engineer.
