were causing or likely to cause death or serious physical harm to
employees in that employee(s)
were exposed to falling objects from damaged storage racks due to the
possibility of rack failure
or collapse.
(a) Location - Warehouse:
On or about 02/20/04, employees were exposed to the hazard of falling
materials stored
overhead due to bent upright supports, mis-aligned horizontal supports,
and lack of
design by qualified persons.
Feasible abatement methods include but are not to the following:
1. Ensure that the rack manufacturer or a qualified person has
predetermined the designated load
sizes and rating.
2. Place plaques within the environment of existing racks designating
their load sizes as required
by ANSI MH16.2-1984 (R1996) For the Use of Industrial and Commercial Steel
Storage Racks.
3. Keep aisles clear; insufficient clearance may result in impacts from
handling equipment
resulting in rack damage.
4. Ensure racks are properly aligned, plumb, and level as per
manufacturers installation
instructions.
5. Implement an inspection program and a periodic planned maintenance to
ensure the integrity
of the racks, and hazard awareness.
6. Ensure employees are following a preventative maintenance and
inspection program for steel
storage racks.
7. Encourage employees to report any damage to racks at time of incident,
specifically damage
due to mechanical equipment.
8. Repair all defective components of storage racks in a timely manner.
