Section 5(a)(1) of the Occupational Safety & Health Act of 1970:  The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees, in
that, employees were
exposed to crushing injuries while inflating the tires on a 2 1/2 cubic
yard front end loader and
not following industry standards for safety procedures when performing
such operations:
Caterpillar 2 1/2 cubic yard front end loader:  Employees who were
required to inflate the tires
on the front end loader were not instructed on the procedures and industry
established practice
for adding air to the tires to avoid injury in the event of tire failure.
The hose used for the
inflation of the front end loader tires did not have an clip-on chuck with
an inline pressure gauge
or regulator exposing employees to serious injury in the event of tire
and/or rim failure.
Feasible and acceptable methods of abatement include:
1.Provide training for employees who inflate the tires on the 2 1/2 cubic
yard front end
loader.
2.Provide a clip-on chuck with an in-line pressure gauge or regulator.
3.Provide multi-piece rim wheel service charts in area where employees
service or inflate
tires mounted on multi-piece rim wheels.iece
