recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazards associated with crushing injuries from presses:
Press Dept.:
The employer did not ensure that the clamps which held the die to the
press plate of a
hydraulic press were secured. An operator's fingers were amputated when
the 1500
pound die fell while making light domes.
Possible methods to abate the hazard are:
1) Institute a formal inspection program to ensure that clamps and bolts
are secured and
do not come loose.
2) Evaluate all die setups to ensure that clamps and bolts  are sized
correctly  for the
weight of the dies.
3) Ensure that the dies of hydrualic presses are adequately secured to the
press plates.
