Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
guard rails could withstand a horizontal force of 200 lbs.
3)The floor surface was not slip resistant.
4)Workers on the platform were not guarded from moving parts of the
forklift truck that represent a hazard.
5)There was no means to assure that the platform was centered laterally on
the forklift truck.
6)The following information was not provided for the platform:
(a)maximum load including personnel and equipment;
(b)weight of empty platform;
(c)minimum capacity of truck on which the platform can be
used.
Among other methods, one feasible and acceptible method to correct this
hazard
is to utilize a self-propelled elevating work-platform in accordance with
the
requirements set forth in ANSI A92.6 or to utilize only platforms designed
for
elevating personnel with forklifts, in accordance with the requirements
set
forth
in ASME B56.1-2000, Safety Standard for Low Lift and High Lift Trucks,
Section 4.17 Elevating Personnel, and Section 7.36 Platforms - Elevating.
