Code of Virginia - 40.1-51.1(a): The employer, Rockingham Public Works, did not furnish a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were operating a hoist that had not had frequent or periodic inspections of all controls and operating mechanisms safety devices, hooks, chains, and chain stretch, that is required by American national Standard Safety Code for Hoists (ANSI) B30.16-1987. In the Central Garage Shop, an American Hydraulic Co. 1500 lbs hoist (Model #FC750; ID #00235) was used by employees. It had not been inspected and no inspection documents were kept. One feasible and acceptable method to correct this hazard is to conduct frequent (monthly) inspections and periodic (annual) inspections of such items as loose bolts, excessive wear of chain, chain stretch, hook, supporting structure, end connections of load chains, all controls and operating mechanisms, hook, chains and chain stretch. The inspections shall include a written, dated, and signed inspection report and record to indicate required inspection have been completed for items inspected inspected during an annual inspection.
