SCRR 71-112A: Failed to furnish a place of employment which is free of
recognized hazards which may cause death or serious physical harm to his
employees and comply with this regulation and other occupational safety and
health rules and regulations promulgated under Chapter 15 of Title 41,
Code of
Laws, State of South Carolina, 1976, as amended, as follows:
A.  Employer knew or should have known that the operator of a Gehl 553
Dynalift brand forklift, Serial Number J0104349, not at the controls while
one
employee was elevated in a work basket in excess of 11 feet high at
southeast
side of building, constituted a hazard of the lift shifting or of the
employee
falling or of the lift being struck by objects or other hazards and no way
for
the employee to get down.  A feasible and useful method to correct this
hazard, among others, is to have the operator remain at the controls while
employees are in an elevated basket.
