A.R.S. 23-403.A.: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employer did not ensure that the lifting device (i.e. lifting beam/spreader bar) met the applicable requirements set forth in ANSI/ASME B30.20-1985. There were no rated load markings and identification plates stating manufacturer's name, serial number, lifter weight and rated load attached. (a) Schon area: A spreader bar used to lift and move rolls of felt did not have the manufacturer's name, serial number, and rated load marked on an identification plate.
