Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a crushing hazard due to defective or missing cutting
levers, grab irons or
riding stirrups and railroad tracks:
(a) Allegheny Ludlum, Brackenridge Plant:  On or about 2/19/04, the cut
lever was
missing on 325 ingot car.  In addition, excess water covered the rail road
track in this
area near the 8-2 soaking area.
(b) Allegheny Ludlum, Brackenridge Plant:  On or about 2/19/04, 320 ingot
car had bent
stirrups.
(c) Allegheny Ludlum, Brackenridge Plant:  On or about 2/18/04, the
company had not
implemented a regular rail inspection program to detect defects such as
submerged rail
beds, out of gauge tracks, separated and/or worn switches, worn splice
plates and frogs
within the in-plant railroad system.  Derailments have occurred on
numerous occasions
(at least 10 times) over the past 2 months.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to implement and enforce an on going car repair program and a
track inspection
program.
