Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer
did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to:
On or about August 9 and August 22, 2003, at the jobsite located at 524
Church Street,
in Jonesboro, Arkansas, employees working from a Grove AMZ 66 and a Genie
S-60
manlifts were exposed to injuries from falls should the machines
malfunction due to the
user not causing an annual inspection to be performed on them. One
feasible method of
abatement, among others is to cause an inspection as outlined in Section
6.6 of the
American National Standards Institute's Standard for Boom-supported
Elevating
Work Platforms. (ANSI A92.5 1992)

On or about August 9 an employee operating and working from a Grove AMZ
66 manlift and on or about August 22, 2003, employees operating and working
from a Genie S-60 manlift at the jobsite located at 524 Church Street, in
Jonesboro, Arkansas were exposed to injuries from falls because the
employer
directed or authorized employees to operate these aerial platforms and did
not
ensure that they had been properly trained. One feasible method of
abatement,among others is to ensure that the operators have been trained in
accordance with the manufacturer's operating and maintenance manual, the
employer's work instructions, and with Section 8 of the American National
Standards Institute Standard, Boom-Supported Elevating Work Platforms.

surface within
the limits specified by the manufacturer. One feasible method of
abatement, among
others is to require the aerial platform to be used in accordance with
Section 7.9(a) of
the American National Standard Institute, Boom-Supported Elevating Work
Platforms (ANSI A92,5-1992)
