Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to recognized hazards of fire, smoke and or explosions: 1.)
the travel distances
from hazardous occupancy area(s) to the outside or to a place of safety
exceeded
75 feet for
assembly, chambon press, and composition room employees. Among others, a
feasible  means
of abatement would be to comply with the National Fire Prevention
Association Life Safety Code
- NFPA No. 101 - 2000, Section 7.11.1, which requires "Where the contents
are classified as
high hazard, exits shall be provided and arranged to permit all occupants
to escape from the
building or structure or from the hazardous area thereof to an outside or
a place of safety with
a travel distance of not more than 75 ft (23 m) measured as required in
7.6.2.

were exposed to crushing injuries, broken bones, and lacerations as the
result of being struck
by rolls of paper weighing up to 2010 lbs each: a) The maximum lift
capacity (3750 pounds) of
the Cascade Paper Roll Clamp, model 45FRCF06B, serial number 90118-01,
attached to
caterpillar forklift serial number A2EC362196, model 2EC30, Type E, was
exceeded when the
operator routinely used the paper roll clamp to lift double rolls of paper
that weighed in excess
of 3750 pounds. Among other methods, feasible and acceptable methods to
correct this hazard:
Comply with the Cascade Operator's Guide for D, F, & G - Series Fixed
Frame Paper Roll
Clamps, Manual Number 210135 R-2 and follow the paper roll data plate
which lists the
maximum lift capacity of the paper roll clamp at 3750 pounds.

employer did not
furnish employment and a place of employment which were free from

were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of contact with hot surfaces:
a.Incinerator Room - On or about March 1,2004, employees were required to
work
around an incinerator in the area of the combustion chamber where the
refractory
had deteriorated which allowed the outside steel walls to get red hot,
causing
metal fatigue and the potential of burns from burn-through at these
locations.
b.Incinerator Room - On or about March 1,2004, employees were exposed to
fire
flashback when dumping barrels of waste match stems into the charging area
of
the incinerator.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to comply with the National Fire Protection Association (NFPA) 82,
entitled "Incinerators and
Waste and Linen Handling Systems and Equipment."  Specifically, the
following paragraphs
provide guidance:
4.2.1.1(4)  The combustion chamber, inner walls, roofs, and curtain walls
shall be
constructed so as to withstand the combustion temperatures involved and
shall maintain
their integrity under all operating conditions;
4.2.1.3  Incinerators shall be designed with internal insulation or
refractory, or shall be
otherwise protected by location, guard rails, or shields such that all
areas or surfaces
normally accessible to personnel shall not exceed 71.1 degrees C (160
degrees F)
4.2.5.1 A waste charging system and appropriate controls shall be provided
to prevent
the direct discharge of flames, combustion gases, and heat from the
incinerator during
waste loading operations.
