Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to hazards of explosions and fire.
a.Auto body/scrap metal shredder area - On or about July 23, 2004
employees were
feeding compressed gas cylinders and vehicle gas tanks into the scrap
yard's 3000
hp metal auto body/scrap metal shredder with inadequate safety procedures
to
prevent exposure to explosion and/fire.
Feasible and acceptable methods of abatement, among others, is 1) to
comply with the Institute
of Scrap Recycling Industries (ISRI) Safety Reference Manual, SHREDDER
section, and post
procedures to prevent the following materials from entering the shredder:
gasoline tanks;
propane tanks; paint cans; batteries; compressed gas cylinders; aerosol
cans;
undischarged
automobile air bags; drums that may contain volatile material; wire, rope,
cable, or long, stringy
items; materials whose hardness exceeds shredder operating capacity. 2)
Repair the two non-
functional video cameras/monitors that give the shredder operator a view
of material being fed
into the shredder and do not operate the shredder should any of the four
video cameras/monitors
become non-functional or blocked.

were exposed to a crushing hazard:
a.) On or about 07/23/2004, throughout the facility and in the employer's
yard
area, an employee was operating a Yale powered industrial truck (serial
number
E17781229T) over rough and varied terrain and the forklift was not equipped
with a seatbelt for the operator (the powered industrial truck was
provided with
ropes).
ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement
method
to correct this hazard is to install a seatbelt for the operator and
require its use.
