Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls to the ground: a.Sterling Hill Lane, Exeter, New Hampshire - The platform used on the Lull lift did not have a 4 inch toe plate. b.Sterling Hill Lane, Exeter, New Hampshire - The platform used on the Lull lift did not have a plate with the following information on it: maximum work load including personnel and equipment; and the weight of the empty platform. c. Sterling Hill Lane, Exeter, New Hampshire - The platform used on the Lull lift was not secured to the vertical face of the forks, carriage or lifting mechanism. ABATEMENT NOTE: One feasible and acceptable method of abatement would be to comply with the requirements of the American Society of Mechanical Engineers document ASME/ANSI B56.6-1992, Safety Standard for Rough Terrain Forklift Trucks.
