being struck by falling pallets:
(a) Pechin Shopping Village, Dunbar, PA: On or about June 23, 2004,
employees were not trained in the safe
operation of the BT Pallet truck.  Employees were loading unsecured empty
wooden pallet skids into a box
trailer when the top two pallets fell, fatally striking the operator of
the BT Pallet truck.
Among other methods, one feasible and acceptable means to correct this
hazard is to train employees in the
safe operation of the BT pallet trucks, in accordance with the
manufacturer's Operator's Manual; inspect the
area and take special care if there are protruding parts from racks,
shelves, and walls that can cause injury;
handle only loads that are secured, stable, safely arranged and evenly
distributed on the forks.
