Reef Chemical Company Inc., 2559 W IH 20, Odessa, TX.  For the period of
time up to and
including 01/18/06, a manager placed an ACRA Electric Company Drum Heater
(Model AGM-
55) on a poly drum of AC-306 (glacial acetic acid).  The heater remained
on the drum overnight
allowing the drum to "melt", resulting in an acetic acid spill.
Feasible and acceptable abatement would be to ensure Drum Heaters are used
in accordance with
manufacturer instructions.

including 01/18/06, piping in the tank farm was not identified as to the
contents or direction of
flow.  Chemicals and chemical mixtures flowing through the piping include,
but is not limited
to, toluene, xylene and methanol.
ANSI A13.1 - 1975, Scheme for the Identification of Piping Systems,
paragraph 3.1 Legend
states "...Positive identification of the content of a piping system shall
be by lettered legend
giving the name of the contents in full or abbreviated form.  Arrows shall
be used to indicate
direction of flow.  Use legend to identify contents exactly, and give
temperature, pressure, and
other details.......Legends shall be applied close to valves and adjacent
to changes in direction,
branches, and where pipes pass through walls or floors, and at frequent
intervals on straight pipe
runs."
Feasible and acceptable abatement would be to identify and label contents
of all piping as per
ANSI A13.1.
