manufacturer's specifications
and limitations applicable to the operation of crane(s) or derrick(s):
(a)Miron Construction Co, Inc. Dells Hydro Plant, 1200 Forest St., Eau
Claire, WI:
Employees were exposed to struck by hazards when the employer did not
ensure
that a mobile hydraulic gantry lift system was installed and operated
properly,
while moving an approximately 60 ton hydroelectric-generating component
(distributor). The lift operation did not follow the hydraulic lift system
operators
manual, which resulted in the collapse of the gantry lift system, damage
to the
distributor being lifted, and injuries to two of their employees.
1.The lift system track was not shimmed every three feet. The 4 Point Lift
Systems
operation manual states:  Always shim runway track every three feet or
less.
Always have a qualified professional engineer verify capacity to span
greater
lengths.
2.The distributor being lifted and moved was not centered. The 4 Point Lift
Systems operation manual states:  If there is any question about the
center of
gravity, or distribution of the load equally among the lifting units,
contact a
qualified engineer for verification of the layout.
3.A concrete anchor bolt was used in place of a shear pin to connect the
propel
cylinder. The manufacturer of the lift system provided shear pins.
4.The lifting units were not plumb and level. During lifting operations,
it was
observed that the wheels of the lift system came off the track. The
operators
manual states: Use accurate levels to make sure the lifting units are
level and
plumb, and WARNING!!! Always keep the system units level and plumb during
lifting.
5.The distributor was raised and lowered with the propel cylinder
connected. This
is contrary to the operators manual which states: After the load is raised
attach
the propel cylinder.
6.The side shift trolley systems were modified by welding angle iron on
one side
of the trolley to maintain a distance of 24" centers. The manufacturer
provided
adjustable spacers which are designed to be pinned to the trolley system.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE MEANS
OF ABATEMENT WOULD BE TO COMPLY WITH THE
MANUFACTURER'S OPERATION MANUAL FOR THE PROPER
INSTALLATION AND OPERATION OF THE EQUIPMENT. FOLLOW ALL
OF THE OPERATION MANUAL CAUTIONS!, WARNINGS!, AND SAFE
LOAD CALCULATIONS.
