Section 5(a)(1) of the Occupational Safety and Health Act of 1970 : The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees and other contractors on site
in that: they were exposed to
crushing injuries and broken bones caused by improper lifting of 82 foot
long truss weighing 684 pounds.
A.Worship Center - The 82 foot wide wood truss were being lifted in place
by connecting a 15
foot wide spreader bar to two slings with a truss hook that attached to
the top cord of the truss
spaced approximately 15 foot apart and a non-lifting 40 foot long metal
stiffner on the bottom
cord of the truss as determined on March 22, 2005.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO
CORRECT THIS HAZARD IS TO FOLLOW THE RECOMMENDATIONS OF THE BCSI - B1
SUMMARY SHEET FOR HANDLING, INSTALLING AND BRACING OF METAL PLATE
CONNECTED WOOD TRUSS, SECTION, HOISTING RECOMMENDATIONS BY TRUSS SPAN
PROVIDED BY THE TRUSS MANUFACTURER AND DEVELOPED BY THE TRUSS PLATE
INSTITUTE TO USE A STIFFBACK OR SPREADER BAR 2/3 TO 3/4 OF TRUSS LENGTH
FOR TRUSS
OVER 60 FOOT IN LENGTH..

A.Worship Center - A spreader bar or stiffback was not used while
attempting to
set the 82 foot long, # 2 truss, which was the first T1 truss, causing the
truss
bottom cord to crack in the middle, as determined on March 22, 2005.

METHOD TO CORRECT THIS HAZARD IS TO FOLLOW THE RECOMMENDATIONS
OF THE BCSI - B1 SUMMARY SHEET PROVIDED BY THE TRUSS
MANUFACTURER AND DEVELOPED BY THE TRUSS PLATE INSTITUTE TO USE A
STIFFBACK OR SPREADER BAR 2/3 TO 3/4 OF TRUSS LENGTH FOR TRUSS OVER
60 FOOT IN LENGTH..

by the lifting of  loads with the Grove RT635C mobile hydraulic crane
before determining
the gross load to be lifted.
a.Worship Center - Crane operators did not the total lift that was being
made
and were depending on the use of a PAT Load Motion Indicator, (an
operational aid) to determine if the lift, consisting of  82 foot wide
wood
truss
which weigh 684 pounds, the use of a 40 foot long metal stiffner on the
bottom cord of the truss and a 12 foot long spreader bar above the truss,
could
be made within the cranes operating radius as determined on March 22,
2005.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS TO DETERMINE THE TOTAL WEIGHT
OF THE LIFT TO BE MADE AND APPROPRIATELY PROGRAM THE PAT LOAD
MOMENT INDICATOR WITH THE CORRECT INFORMATION PRIOR TO
ATTEMPTING THE LIFT.PTING
