Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to flying objects and the release of high pressure:
a.Viking Well Service, Inc. - Employees were using a flexible hose
designed for
"Air and Water" to release gas pressure and chemicals such as hydrochloric
acid from the gas well.  The flexible hose was not designed for the
operating
conditions to which it was exposed, as determined on 10/25/05.
NOTE: Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to: Use pumps, piping, hoses, and pressure relief devices that
are designed to meet
the requirements of the operating conditions to be encountered.
