Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to crushing and struck by injuries due to the
deficiencies noted on
the Drott 650B travelift crane:
a.Drott 650B 40 ton, 1974 travelift yard crane serial # 6209429 -  Adequate
frequent and regular inspections were not made of this crane, as evidenced
by
the following deficiencies:
(1) The Drott crane was not equipped with wheel guards;
(2) The Drott crane did not have a operable horn / alarm;
(3) The Drott crane had excessive oil and hydraulic fluid leaks around
the motor and left steering arm;
(4) The Drott crane had one bolt missing from the front area where the
right horizontal beam connects to the right vertical beam.
(5) The Drott crane, There were no records of monthly inspection of
the cable and hooks.
(6) The 45 foot long spreader beam was not labeled as to
its rated load capacity.
"AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS: follow the manufacturer recommendations
and the requirements of AMSE B30.17 and ensure that a qualified person
performs
inspections and maintains hoisting equipment in accordance with the
manufacturer's
information and guidelines. The spreader beam needs to be rated per the
requirements of
AMSE B30.20"
