Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: (a) Assembly Area - Twelve fastener (eyelet) machines were not provided with single cycle controls to protect employees from unexpected successive strokes, exposing operators to cushing and laceration hazards. Among other methods, one feasible and acceptable method to correct this hazard is to include, but not be limited to the following: (1) Conform to the American National Standard Institute B154.1 - 1984. In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: (a) Assembly Area - On or about September 19th, 2003, the foot pedals on various rivet and eyelet fastener machines were not provided with: 1) Protection against unintended operation from falling or moving objects and from someone accidentally stepping on the control. 2) Pads with non-slip contact surfaces attached. Among other methods, one feasible and acceptable method to correct this hazard is to include, but not be limited to the following: (1) Conform to the American National Standard Institute B154.1 - 1984. In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
