ladder without
the filler blocks inserted between the rungs to support the cleats. The
employees were exposed
to a fall hazard when they used the ladder.

hazard is to include,
but not limited to the following:
(1) Conform to the American National Standard Institute (ANSI) A14.4-1992,
section 4.5.2
