Nevada Revised Statute 618.375(1):  The employer did not furnish
employment and a place of
employment which was free from recognized hazards that were causing or
likely to cause death
or serious physical harm to employees:
The employer failed to ensure that an employee was protected while working
from a Sky Trak
Forklift as follows:
(a)At the west side of the building, an employee was using a pneumatic
nail gun
while standing on a 4' by 8' sheet of plywood that was sitting on the
forks.  The
employee was exposed to a fall hazard of approximately 8 feet.
Reference Sky Trak Owners/Operators Manual: Section 5.  Carrying
Personnel:
Use only an approved work platform for lifting and lowering personnel.
Never
transport personnel in a work platform for even the shortest distance.
Serious
injury or death can occur if these rules are not obeyed.  Riders can fall
and
be
crushed or run over.
One feasible means of abatement is to make sure that employees follow the
manufacturer's operations manual requiring only an approved work platform
is
used.
