Nevada Revised Statute 618.375(1):  The employer did not furnish
employment and a place of
employment which was free from recognized hazards that were causing or
likely to cause death
or serious physical harm to employees:
The employer did not ensure that a subcontractor employee was protected
from falling as
follows:
a.At the southwest side of the building, an employee was working
from the bucket of a Grove A45EJ aerial lift without being tied-
off.  The employee had used this lift and another model of aerial
lift during the previous week as well, operating them to reach
heights of approximately 10-20 feet.

Section 8.8:  During operation.  The aerial platform shall be used
in accordance with this standard.
Section 8.8(g):  Ensure all personnel in the platform are wearing
fall protection devices and other safety gear as required at all
times.
One method of abatement is to ensure that subcontractor employees were
trained/retrained on
the proper wear and anchorage of personal fall protection equipment in
accordance with ANSI
92.5 and the operator's manual for each lift before operating the lift.
