Minn. Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees:and as referenced in ANSI B56.6 1992 Safety Standard for Rough Forklift Trucks. Section 8.25 Platform for Elevating Personnel artical 8.25.1 (b) states that floor dimensions which shall not exceed two times the load center distance listed on the rough terrain forklift nameplate, measured parallel to the longitudinal center planeof the truck, nor have a width greater than the overall width of the truck [measured across the load bearing tires plus 10 inches (250 mm) on either side]. Minimum space for each person on the platform shall not be less 18 inches (450 mm)in either direction. Two employees were exposed to fall hazards of approximately 16 feet to ground level while they were working in a basket attached to a forklift without a personal fall arrest system. Also, the basket they were working in extended out past the load-bearing wheels by approximately 4 feet on each side, which could have inadvertently tipped over when employees and/or materials were placed along the end of the basket.
